Step 1
Risk Stratification / Inclusion Criteria
Payor Data Query to Identify Population.
Step 2
Home Evaluation & Care Plan Assignment
2 Primary Criteria Needed.
Step 3
Severity Staging
Very Severe, Severe, Moderate, Mild — Care Term Authorization.
Step 4
Monthly Home Visits
Based on patient risk stratification, patient will be seen 1 to 2 times a month by respiratory coach.
Step 5
Case Conferences
Regular evaluation by RT will determine patients breathing status — Improvement, Stability, and Decline.
Step 6
Data Collection
Patients will have reported outcomes on a monthly basis showing breathing status and readmissions data.
